MAH contact details form

Medicines verification institute Slovenia (ZAPAZ) is a non-profit organization (NMVO) responsible for implementing and operating the national medicines verification system (NMVS) in Slovenia.

As such we are contacting all marketing authorization holders, who are distributing medicines on Slovenian market.

ZAPAZ is currently preparing contractual cooperation agreement for operating NMVS. Since we don’t have complete information in our register, we are kindly asking you to provide us contact information of your formal representative who will represent your organization in further steps.  Please send us the following contact details:

Full name of the company which will sign the agreement with ZAPAZ
Name and surname of the contact person
Please input the international phone number of the representative person
Please input the address of the Marketing Authorization Holder's seat (Country, Postal code, Town, Street address)
VAT number of the MAH which will sign the agreement
Registration number of the MAH which will sign the agreement
Please state all MAHs in Slovenia, including their VAT and registration numbers
